Physical Packets for 2019-2020 Available
Physical packets for the 2019-2020 school year are now available online and can be downloaded via the links below.
Physical Packet - Section 1-6 and Insurance information sheet.
The physical packet for athletes must be completed prior to the start of the student/athlete's first legal day of practice and must be filled in completely.
In addition, other physical forms/sections are available for various reasons and they are addressed below.
Section 7 - This is for any student who suffers no injuries and the parents/guardians certify this. This is to be used only between the fall and winter seasons, and the the winter and spring seasons. Do not fill this form out during the 2014 fall season.
Section 8 - This is for any student that has been identified as "injuried" during the sports season and that we, the Jefferson-Morgan School District". are requiring them to get another physical for the purpose of participating in one of our programs.
Section 9 - This is the Wrestling CIPPE Form, which will be filled out by our trainer prior to the wrestling season. Our medical staff will be filling this form out for that particular sport.
